KARAKTERISTIK PASIEN RINOSINUSITIS KRONIK PRIMER DAN SEKUNDER DI RS DR. MOHAMMAD HOESIN PALEMBANG PERIODE 2023-2024

Chronic rhinosinusitis (CRS) is a clinical syndrome characterized by symptomatic inflammation of the nasal cavity and paranasal sinus mucosa lasting for more than 12 weeks. CRS is classified into two types, primary and secondary CRS. Primary CRS refers to a persistent inflammation without an identifiable etiopathogenic factor, while secondary CRS develops as part of another pathological process in which the sinus inflammation represents a manifestation of the underlying disease. This study was conducted to determine the characteristics of patients with primary and secondary chronic rhinosinusitis at Dr. Mohammad Hoesin General Hospital Palembang during the 2023–2024 period. This study is a descriptive research with a cross-sectional design using a total sampling technique. Data were analyzed univariately using the Statistical Package for the Social Sciences (SPSS) version 29. From a total of 298 samples, 178 patients (59.7%) were diagnosed with primary CRS and 120 patients (40.3%) with secondary CRS. The highest incidence of primary CRS was observed in the 31–40-year age group (21.3%) and was more common in females (51.7%), while secondary CRS was most frequent in the 41–50-year age group (24.2%) and also predominated in females (57.5%). The main clinical symptom in both groups was nasal obstruction (primary CRS 96.1%; secondary CRS 87.5%). The most common risk factor for primary CRS was septal deviation (68.5%), whereas for secondary CRS it was dental infection (39.2%). Anatomical distribution showed a predominance of bilateral involvement in primary CRS (84.3%) and unilateral involvement in secondary CRS (100%). The dominant endotype in primary CRS was type 2 inflammation (56.2%), while secondary CRS was entirely characterized by a local pathological endotype (100%). The most frequent clinical phenotype in primary CRS was CRSwNP (53.4%), whereas in secondary CRS it was tumour (48.3%). The most commonly affected sinus in both groups was the left maxillary sinus (primary CRS 86%; secondary CRS 56.7%). Primary CRS was found to be more common than secondary CRS. Both types were predominantly observed in females of productive age, with nasal obstruction as the main clinical symptom. Primary CRS tended to present bilaterally with the CRSwNP phenotype, whereas secondary CRS was generally unilateral with the tumour phenotype.
