Spinal anesthesia for lower extremity surgery often causes complications such as back pain. Body mass index (BMI) is suspected to influence the risk of back pain, but studies on this relationship are still limited, especially at RS Mohammad Hoesin Palembang. This was a quantitative observational analytic study with a cross-sectional design. The sample consisted of postoperative lower extremity patients who received spinal anesthesia and met the inclusion criteria. Primary data were collected and analyzed using the chi-square test to assess the relationship between BMI and back pain. Of the 53 patients, most were aged 18–44 years (47.2%) and male (58.5%). The majority had a normal BMI (47.2%) and were classified as ASA II (56.6%). All patients received ketorolac as postoperative analgesia. Two days after surgery, mild back pain was the most common finding (41.5%). A significant relationship was found between BMI and the occurrence of postoperative back pain (p < 0.05). There is a significant association between body mass index and back pain in postoperative lower extremity surgery patients who received spinal anesthesia at RS Mohammad Hoesin Palembang.
