Background: Iron deficiency is a common complication in hemodialysis patients and has been known to have a negative impact on cognitive function. This study aimed to analyze the association of iron deficiency with cognitive impairment in chronic hemodialysis patients. Methods: This observational study was conducted using cross-sectional design in chronic hemodialysis patients at Mohammad Hoesin Hospital Palembang, a top referral hospital for Southern Sumatera Region in Indonesia. Sample was acquired using purposive sampling and interviewed with the MoCA-INA questionnaire. Data were analyzed using univariate, bivariate (Fisher's exact test), and multivariate (logistic regression) analyses with SPSS ver. 27 Results: A total of 79 patients were enrolled, consisting of 15 iron deficiency and 64 non-iron deficiency patients. Bivariate analyses showed a significant association between iron deficiency and cognitive impairment (p=0,030). Multivariate analyses demonstrated that age (p=0,033) and history of diabetes (p=0,035) were independent factors of cognitive impairment. Iron deficiency does not show a statistically significant result after analyzed with other confounding factors (p=0,133). Conclusion: Iron deficiency is associated with cognitive impairment in chronic hemodialysis patients. Age and history of diabetes were independent factors of cognitive impairment in chronic hemodialysis patients.
