Background: Medical students face a heavy academic workload, high performance expectations, and psychological pressure that may affect their mental health. Unmanaged academic stress can lead to future anxiety, particularly regarding career readiness and academic achievement. Methods: This quantitative study used a cross-sectional design involving 298 active medical students selected through total sampling. The instruments used were the Academic Stress Scale (ASS) to measure academic stress and the Future Anxiety Scale (FAS) to measure future anxiety. Data were analyzed descriptively and inferentially using the Fisher’s Exact Test with a significance level of 0.05. Results: The descriptive analysis showed that most respondents were in the low category of academic stress (54.7%) and in the high category of future anxiety (72.8%). The Fisher’s Exact Test indicated a significant relationship between academic stress and future anxiety, with a Pearson p-value = 0.000 (p < 0.05). The distribution of variables X and Y demonstrated that students with high levels of academic stress also tended to have high levels of future anxiety. Conslusion: There is a significant influence between academic stress and future anxiety among medical students at the Faculty of Medicine, Universitas Sriwijaya.
