Background: Coronary Artery Disease (CAD), particularly Acute Coronary Syndrome (ACS) with ST-Elevation Myocardial Infarction (STEMI), is a leading cause of morbidity and mortality worldwide. Coronary artery occlusion in STEMI patients disrupts blood flow to the myocardium, triggering clinical symptoms such as angina, dyspnea, arrhythmias, and cardiogenic shock. This study aims to analyze the relationship between the location of coronary artery occlusion (Left Anterior Descending Artery / LAD, Left Circumflex Artery / LCX, and Right Coronary Artery / RCA) and clinical symptoms in STEMI patients treated at RS KEMENKES Dr. Mohammad Hoesin Palembang. Methods: This is an observational analytical study with a cross-sectional design. Primary data were collected through medical examinations of STEMI patients undergoing treatment at RS KEMENKES Dr. Mohammad Hoesin Palembang between August and October 2025. Bivariate analysis was conducted to evaluate the relationship between the location of coronary artery occlusion and clinical symptoms, such as Killip score, angina, dyspnea, arrhythmia, and cardiogenic shock. Results: Of the 78 patients studied, the majority were aged 40-59 years (48.7%) and 76.9% were male. The prevalence of occlusion in LAD, RCA, and LCX was 48.7%, 43.6%, and 11.5%, respectively. Bivariate analysis revealed that LAD occlusion was significantly associated with Killip score II-IV (p=0.026). RCA occlusion was significantly associated with Killip score II-IV (p=0.008). LCX occlusion wasn’t significantly associated with Killip Score. Conclusion: Occlusion in LAD and RCA has a significant relationship with the worsening condition of STEMI patients, as reflected in Killip score, angina, arrhythmia, and cardiogenic shock. While LCX occlusion is significantly related to angina and dyspnea, it does not show a significant effect on Killip score or other symptoms.
