KARAKTERISTIK PASIEN DAN CAIRAN PLEURA PADA KASUS EFUSI PLEURA DI RS DR. M. HOESIN PALEMBANG TAHUN 2024

Background: Pleural effusion is a common clinical manifestation encountered in internal medicine and pulmonology practice, with various etiologies, both transudative and exudative. In Indonesia, the most frequent causes of pleural effusion are tuberculosis, followed by malignancy and heart failure. Analysis of patient characteristics and pleural fluid is essential to determine the type of effusion and assist in establishing the diagnosis. Objective: This study aimed to determine the characteristics of patients and pleural fluid in pleural effusion cases treated at Dr. Mohammad Hoesin General Hospital, Palembang. Methods: This was a descriptive observational study with a cross-sectional design conducted in the Medical Record Department of RSUP Dr. Mohammad Hoesin from July to September 2025. Secondary data were obtained from medical records of pleural effusion patients hospitalized between January 1 and December 31, 2024, who met the inclusion criteria. A total of 64 patients were included using purposive sampling. The analyzed variables comprised patient characteristics, pleural fluid characteristics, and disease diagnosis, which were processed descriptively using univariate and bivariate analyses. Results: Most pleural effusion patients were under 65 years old (71.2%) and male (59.1%). The majority had pleural fluid volumes of 10–20 mL (81.8%), yellowish color (60.6%), and cloudy appearance (50%). Nearly all fluids were odorless (97%) and had a specific gravity 500 (57.6%), and PMN:MN ratio 60 mg/dL in 68.2%, and LDH >320 U/L in 54.5%. LDH ratio >0.6 occurred in 78.1% and protein ratio >0.5 in 63.3%. Based on Light’s criteria, 81.8% of cases were exudative. The most common diagnoses were “others” (39.4%), followed by tuberculosis (25.8%) and malignancy (18.2%). Most exudative effusions were caused by tuberculosis (29.6%) and other diseases (38.9%). Conclusion: Most pleural effusion cases at Dr. Mohammad Hoesin General Hospital Palembang were exudative, with tuberculosis as the predominant cause. These findings highlight the importance of comprehensive evaluation of pleural fluid characteristics to determine etiology and guide appropriate management.
