ASUHAN KEPERAWATAN PADA PASIEN CHRONIC KIDNEY DISEASE DAN APLIKASI TERAPI RELAKSASI OTOT PROGRESIF PADA MASALAH KEPERAWATAN GANGGUAN POLA TIDUR

Introduction: Chronic Kidney Disease (CKD) is a chronic kidney disease that causes the accumulation of fluids, electrolytes, and waste in the body, which affects sleep patterns. In nursing interventions, the use of sleep support in the form of Progressive Muscle Relaxation (PMR) is a technique performed by contracting and stretching muscles to provide relaxation effects and address sleep disturbances. Aims: Describing the implementation of nursing care for CKD patients and applying PMR therapy to address sleep pattern disturbances. Method: Case study with 3 CKD patients in the inpatient room of RSMH Palembang. Results: The assessment data found in all three patients revealed that they complained of pain, difficulty sleeping, frequent awakenings, and only getting 3-4 hours of sleep, resulting in inadequate rest. Additionally, there was a decrease in hemoglobin concentration and an increase in blood pressure. The nursing diagnoses identified were 5 actual diagnoses related to acute pain, ineffective peripheral perfusion, disrupted sleep pattern, hyperthermia, and ineffective breathing pattern, along with 1 risk diagnosis related to the risk of infection. Discussion: The main interventions provided by pain management include circulation care, rest and sleep support, hyperthermia management and medication administration, infection prevention, respiratory pattern management, and monitoring of respiration, as well as supportive interventions for ROP therapy. Implementation involves dependent, independent, and interdependent actions. ROP application is given for approximately 15-20 minutes daily for 3 days. Relaxation response occurs due to stimulation of the parasympathetic autonomic nervous system nuclei raphe, leading to changes that can control autonomic nerves and stimulate alpha brain waves, making it easier to fall asleep. Conclusion: After receiving nursing care for 3 days, nursing problem 4 is resolved and 2 is partially resolved. The ROP application can be performed well and correctly by the patient, allowing them to do it independently both in the hospital and at home.
