Cholelithiasis is the formation of gallstones due to an imbalance between cholesterol, bile salts, and phospholipids. Factors such as age, gender, and body mass index (BMI) play important roles in this process. Older age is associated with decreased activity of the enzyme 7α-hydroxylase, which reduces bile acid production, while higher estrogen levels in females increase cholesterol secretion into bile. Moreover, obesity can increase the activity of the enzyme HMG-CoA reductase, which accelerates hepatic cholesterol synthesis. This study aimed to determine the association between age, gender, and BMI with total cholesterol levels in cholelithiasis patients undergoing laparoscopic cholecystectomy at Dr. Mohammad Hoesin General Hospital Palembang. This study used an analytic observational design with a cross-sectional approach. Data were obtained from the medical records of cholelithiasis patients who underwent laparoscopic cholecystectomy at Dr. Mohammad Hoesin General Hospital Palembang from January 1, 2022, to December 31, 2024. Independent variables included age, gender, and BMI, while the dependent variable was total cholesterol level. Data were analyzed using the Chi-Square test with a significance level of p < 0.05. Most cholelithiasis patients were female and aged over 40 years. The analysis showed no significant association between age (p = 0.148) or gender (p = 0.237) and total cholesterol levels. However, there was a significant association between BMI and total cholesterol levels (p = 0.032; Cramer’s V = 0.233), indicating a weak to moderate relationship. There is a significant association between body mass index and total cholesterol levels in cholelithiasis patients undergoing laparoscopic cholecystectomy, whereas age and gender show no significant association.
