KARAKTERISTIK PASIEN PARKINSONISME DENGAN FREEZING OF GAIT DI RS MOHAMMAD HOESIN PALEMBANG

Freezing of gait (FOG) is a symptom characterized by a sudden inability to step during walking, described as feet feeling glued to the floor, particularly when initiating gait, turning, or facing obstacles. This symptom commonly accompanies parkinsonism and potentially increases fall risk and decreases quality of life. This study aimed to describe the characteristics of FOG in parkinsonism patients based on age, sex, disease duration, and levodopa equivalent daily dose (LEDD). This descriptive observational study with total sampling method was conducted at the Neurology Polyclinic of Mohammad Hoesin Hospital Palembang from August 6 to September 26, 2025. Twenty parkinsonism patients consisting of 13 PD and 7 unidentified parkinsonism patients were assessed for FOG severity using MDS- UPDRS III item 11. Results showed that 11 patients experienced FOG, with high prevalence in unidentified parkinsonism (85,7%) compared to PD (38,5%). FOG severity distribution in PD was dominated by normal category (61,5%), followed by slight (15,4%), mild (15,4%), and moderate (7,7%). In unidentified parkinsonism, slight category was predominant (71,4%), followed by normal (14,3%) and mild (14,3%). No severe FOG was found in either parkinsonism classification. All patients with FOG were aged > 60 years, predominatly male (PD 80%; unidentified parkinsonism 83,3%), had disease duration ≤ 5 years (PD 80%; unidentified parkinsonism 100%), and LEDD 300-600 mg (PD 80%; unidentified parkinsonism 66,7%). FOG is more prevalent in unidentified parkinsonism but with lower severity than PD. Early detection and comprehensive management integrating pharmacological and non-pharmacological approaches are essential for all parkinsonism types.
