GAMBARAN KADAR LAKTAT PLASMA PASIEN SEPSIS DI INTENSIVE CARE UNIT RSUP DR. MOHAMMAD HOESIN PALEMBANG PERIODE JANUARI 2022–DESEMBER 2024
Introduction: Sepsis is a life-threatening organ dysfunction, one of the leading cause of morbidity and mortality worldwide, including Indonesia. The rapid progression of the disease and its variable clinical manifestations affect sepsis- related mortality. Biomarker testing, particularly plasma lactate level, plays a vital role as an indicator of tissue hypoxia, a predictor of mortality, and a guide for therapy. Understanding the overview of plasma lactate levels in sepsis patients in the Intensive Care Unit (ICU) of Dr. Mohammad Hoesin Palembang Hospital is important to determine the prognosis and effective therapeutic strategies. Methods: This study is a descriptive retrospective observational study with a cross- sectional study design, using a secondary data from the medical records of sepsis patients in the ICU. Data were collected from the medical records unit of Dr. Mohammad Hoesin Palembang Hospital during the period of January 2022– December 2024 using total sampling method. Results: This study analysed data from 219 sepsis patients in the ICU. The mean plasma lactate levels on day one was 2.76 ± 2.23 mmol/L, while on day three it improved to a mean of 2.14 ± 1.53 mmol/L. The sources of infection were dominated by 81 patients (37%) with gastrointestinal tract infections and 77 patients (35.2%) with respiratory tract infection. During the third day of care, the urinary tract infection group showed the lowest plasma lactate levels whilst the respiratory infection group showed a relatively non-significant decrease. Conclusion: Sepsis patients in the ICU of Dr. Mohammad Hoesin Palembang Hospital demonstrated a decrease of lactate plasma levels during the third day of care, with the most common source of infection being the gastrointestinal dan respiratory tracts. These findings reinforce the role of plasma lactate measurement in evaluating therapy and stratifying risk in sepsis patients.
