UJI SENSITIVITAS DAN SPESIFISITAS MINI-COG TERHADAP MOCA-INA SEBAGAI ALAT SKRINING FUNGSI KOGNITIF PADA LANSIA DI POSYANDU LANSIA

The increasing number of elderly individuals in Indonesia has led to a higher risk of cognitive impairment, which can reduce their independence and quality of life. Early detection is necessary so that interventions can be conducted promptly. On the other hand, the use of the Indonesian version of the Montreal Cognitive Assessment (MoCA-Ina) as a standard screening tool requires more time and trained personnel, making it less practical for primary care settings. The Mini-Cog was developed as a faster and easier-to-use alternative. However, evidence regarding its accuracy compared to MoCA-Ina in the elderly population in Indonesia remains limited. This study aimed to assess the accuracy of the Mini-Cog compared to MoCA-Ina in detecting cognitive impairment among the elderly at integrated health service posts for the elderly (posyandu lansia). This research was an observational study with a cross-sectional design, conducted as a diagnostic test. A total of 95 elderly individuals aged ≥60 years participated as respondents and underwent both the Mini-Cog and MoCA-Ina assessments. Analyses were performed to determine the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), accuracy, and the area under the curve (AUC) of the Mini-Cog against MoCA-Ina. The results showed that the Mini-Cog had a sensitivity of 78.6%, specificity of 84%, positive predictive value of 93.22%, negative predictive value of 58.33%, and an overall accuracy of 80%. An AUC value of 0.855 (p < 0.001) indicates good discriminative ability in distinguishing between the elderly with and without cognitive impairment. Based on these findings, the Mini-Cog has diagnostic value that is not substantially different from MoCA-Ina and can be used as a rapid, simple, and practical screening tool for detecting cognitive impairment in the elderly at the primary care level, such as integrated health service posts for the elderly.
