PROFIL PASIEN PENGIDAP PENYAKIT JANTUNG TIROID DI RSUP DR. RIVAI ABDULLAH PERIODE JANUARI-DESEMBER 2024

Background. Thyroid heart disease (THD) is a cardiovascular disorder arising from thyroid dysfunctionhyperthyroidism or hypothyroidismwhich may cause palpitations and arrhythmias associated with systolic or diastolic dysfunction. This may progress to complication such as heart failure and increased mortality. Epidemiological data in Indonesia remains limited. Therefore, this study aimed to delineate the profile of THD patients at Dr. Rivai Abdullah Central General Hospital during January-December 2024. Methods. This descriptive observational study utilized secondary medical record data. Samples were obtained through total sampling, consisting of 70 patients. Variables included sociodemographic characteristics, thyroid hormone abnormalities, blood pressure, associated symptoms and diagnoses, antithyroid medication, and results of routine blood tests, electrocardiography, radiology, and echocardiography. Frequency distribution analysis was performed using SPSS version 27. Results. Most patients were aged 41-60 years (50%) and predominantly female (87,1%) with basic education (45,7%) and unemployed status (74,3%). Hyperthyroidism was the predominant dysfunction (91,4%) and stage 2 hypertension was the most frequent blood pressure category (40%). Patients exhibited multisystem symptoms and diagnoses. Electrocardiography revealed sinus tachycardia in 45,7%, while radiology identified cardiomegaly or pulmonary edema (37,8%). Propylthiouracil (PTU) was the most commonly prescribed antithyroid medication (64,3%). Routine blood tests were performed in 71,4% of patients, generally yielding normal results. Among the 27,1% who underwent echocardiography, most demonstrated HFpEF (73,7%). Conclusion. Most patients were women aged 41-60 years with predominant hyperthyroidism, stage 2 hypertension, sinus tachycardia, radiological abnormalities, HFpEF, and PTU as the primary therapy.
