Background. Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterized by inflammation and multi-organ involvement. Its management requires a long-term multidisciplinary approach, which poses challenges for patients to maintain medication adherence. Poor adherence often leads to inadequate disease control, as reflected by higher disease activity, and worsened prognosis. This study aimed to examine the association between medication adherence and disease activity in patients with SLE. Methods. This study employed an analytic observational cross-sectional design. Primary data on medication adherence were collected through interviews using the MASRI questionnaire, while secondary data from medical records assessed disease activity using MEX-SLEDAI at Dr. Mohammad Hoesin General Hospital, Palembang. A total of 40 patients were selected through purposive sampling. The correlation between medication adherence and disease activity was analyzed using Pearson’s test. Results. All respondents were female (100%), predominantly aged 18–40 years (80%). Over half had been diagnosed with SLE for less than two years (52.5%), and most had recent follow-up within 1–3 months (47.5%). The majority showed good medication adherence (70%) and mild disease activity (60%). Bivariate analysis revealed a significant moderate correlation between medication adherence and disease activity (p = 0.004; r = 0.443).
