STUDI DESKRIPTIF GANGGUAN ASAM BASA PADA PASIEN YANG DIRAWAT DI INTENSIVE CARE UNIT (ICU) RS MOHAMMAD HOESIN PALEMBANG TAHUN 2024

Background: Acid-base disorders are commonly found in patients treated in intensive care units (ICU) due to multiple organ dysfunction, and can worsen patient morbidity and mortality. Methods: In this descriptive observational study with cross-sectional design and uses purposive sampling to determine the profile of acid-base disorders in patients admitted in the ICU at RSMH in Palembang in 2024. Results: A total of 61 study subjects, the mean age was 53,62 ± 15,97 years, most of the patients were male, surgical patients (67,2%). The median CCI score of 1 (0 7). The most frequently observed acid–base disorder was primary metabolic acidosis with secondary respiratory acidosis (39.3%). Meanwhile, the predominant primary acid base disorder was metabolic acidosis (55.7%). Arterial blood gas (ABG) analysis and laboratory tests showed pH 7,29 (6,93–7,56), PaCO2 31,50 (16,30–75.50) mmHg, HCO3 15,30 (6,20–39,10) mmol/L, Na 138,10 (125–168,60) mmol/L, Cl 109,95 ± 6,27 mmol/L, TCO2 16,40 (6,70–41,40) mmol/L, and albumin 2,66 ± 0,81 g/dL. In this study, found that metabolic acidosis with elevated anion gap (85,3%) dan 77% patients did not survive, while 23% survived. Conclusion: Patients who were admitted in the RSMH ICU in 2024 with acid-base disorders in 2024 had a mean age of 53,62 ± 15,97 years, were predominantly male, surgical patients, had a median CCI score of 1 (0-7), and had a mortality outcome.
