HUBUNGAN FAKTOR RISIKO ADENOMIOSIS PADA PASIEN ENDOMETRIOSIS DI RSUP DR. MOHAMMAD HOESIN PALEMBANG
Background: Adenomyosis is a condition in which endometrial tissue is present within the myometrium and can cause pelvic pain, abnormal bleeding, and fertility problems. The mechanism of adenomyosis development is not fully understood, but several risk factors have been associated with it. This study aimed to determine the relationship between age, age at menarche, parity, menstrual cycle length, body mass index (BMI), and history of uterine surgery with the occurrence of adenomyosis in endometriosis patients at RSUP Dr. Mohammad Hoesin Palembang during 2022–2024. Methods: This study is an analytical observational study with a case–control design using secondary data from medical records of endometriosis patients at RSUP Dr. Mohammad Hoesin Palembang for the period January 2022–December 2024. The final sample comprised 189 patients, including 62 cases (adenomyosis) and 127 controls (without adenomyosis) who met the inclusion and exclusion criteria. Bivariate analysis was performed using the chi-square test or Fisher’s exact test where appropriate. Multivariate analysis was performed for variables that are significantly associated in bivariate analysis. Results: Analysis showed that age ≥35 years (p = 0.022), history of uterine surgery (p = 0.001), history of cesarean section (p = 0.0029), history of dilation and curettage (p = 0.0014), and history of myomectomy (p = 0.0062) were significantly associated with the occurrence of adenomyosis. Variables such as age at menarche, parity, body mass index, and menstrual cycle length did not show a significant association (p > 0.05). Conclusion: Age ≥35 years and a history of uterine surgery namely cesarean section, dilation and curettage (D&C), and myomectomy are risk factors significantly associated with the occurrence of adenomyosis in patients with endometriosis. The risk factor with the greatest impact on the occurrence of adenomyosis in these patients was a history of D&C (OR = 13.00), followed by a history of myomectomy (OR = 7.73) and a history of cesarean section (OR = 3.93).
