Background: Many problems of ineffective airway clearance are found in patients with lung cancer due to the accumulation of excessive secretions in the respiratory tract. This condition occurs due to airway obstruction, increased mucus production, and decreased ability to cough effectively, thus inhibiting the process of oxygen and carbon dioxide exchange in the lungs. Accumulation of secretions that are not removed properly can cause hypoxemia, shortness of breath, and the risk of secondary infections such as pneumonia. Objective: Provide chest physiotherapy interventions and effective coughing exercises in lung cancer patients at Dr. Mohammad Hoesin Hospital, Palembang with nursing problems of ineffective airway clearance. Method: The method used is a case study by selecting patients who have a diagnosis of lung cancer who are being treated in the komering Rawas 1.1, Rambang 2.1 and Lematang 2 at Dr. Mohammad Hoesin Hospital, Palembang. Results: There was an increase in airway clearance seen in the three patients, characterized by decreased sputum production, decreased dyspnea and improved respiratory rate. Discussion: Chest physiotherapy and effective coughing exercises successfully helped overcome most of the problems of ineffective airway clearance and supported the achievement of nursing goals. The author believes that effective coughing exercises and chest physiotherapy are important forms of non-pharmacological interventions to help patients with pulmonary cancer maintain airway clearance, improve pulmonary ventilation, and prevent complications such as airway obstruction due to secretion buildup. Conclusion: After being given chest physiotherapy and effective coughing exercises for 3 consecutive days, the problem was partially resolved and the airway clearance of the three pulmonary cancer patients improved.
