Introduction : Low back pain (LBP) is a global health problem that significantly impacts quality of life. LBP can be classified as inflammatory or non-inflammatory. Recently, the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) have been studied as practical, accessible, and economical inflammatory biomarkers, potentially supporting evaluation of inflammatory processes in LBP. This study aims to analyze the correlation between NLR and PLR with LBP. Method : This analytic observational study with a cross-sectional approach was conducted at the Rheumatology Clinic of Mohammad Hoesin Hospital Palembang from September to October 2025. Forty-one patients with LBP were included using consecutive sampling. Data were collected through interviews and review of medical records. Statistical analyses used the Mann-Whitney test and Spearman correlation test with a significance level of p < 0.05. Result : Most participants were female (90.20%), aged over 40 years (58.50%), and housewives (46.34%). Non-inflammatory LBP was more common (53.70%). The median PLR in inflammatory LBP was significantly higher than in non-inflammatory LBP (224.02 vs 140.76; p = 0.041). In contrast, the difference in NLR values was not significant (p = 0.133). Spearman analysis showed a weak but significant negative correlation between PLR and LBP (r = - 0.322; p = 0.040), whereas NLR did not show a significant correlation. Conclusion : PLR demonstrated a significant correlation with LBP, indicating its potential as an inflammatory biomarker in disease assessment, while NLR was not correlated. Further research is required to strenghten these findings.
