PROFIL DAN OUTCOME JANGKA PENDEK PASIEN YANG MENJALANI OPERASI HIPOSPADIA DENGAN TEKNIK GLANULAR FRENULAR COLLAR OLEH SINGLE OPERATOR DI PALEMBANG TAHUN 2025
Introduction: Hypospadias is a complex congenital anomaly with a prevalence of 1:250-1:300 births. Glanular-Frenular Collar (GFC) technique emphasizes tissue preservation and formation of near-normal anatomy. This study evaluates the profile and short-term outcomes of hypospadias surgery using GFC by a single operator in two healthcare facilities in Palembang. Methods: A descriptive-analytic quantitative cross-sectional study of 21 male children with hypospadias who underwent GFC surgery by single operator at RSUP dr. Mohammad Hoesin Palembang and Siloam Hospitals Palembang (January 2024-July 2025). Single operator ensures standardization of surgical technique and minimizes interoperator variability. Outcome evaluation was performed using Hypospadias Objective Scoring Evaluation (HOSE) for objective functional assessment and modified Pediatric Penile Perception Score (PPPS) for aesthetic satisfaction of patients/families. Data were analyzed using univariate descriptive statistics and Fisher exact test. Results: The majority of patients (95.2%; n=20) were operated at suboptimal age (>18 months). Hypospadias distribution was predominantly distal (71.4%; n=15) compared to glandular (28.6%; n=6). Functional outcomes showed 76.2% (n=16) achieved good outcomes with HOSE score ≥14, while 23.8% (n=5) had suboptimal outcomes. Aesthetic satisfaction was very high with 90.5% (n=19) very satisfied and 9.5% (n=2) satisfied. Short-term complications occurred in 23.8% (n=5) patients, while 76.2% (n=16) had no complications. Relationship analysis showed no significant association between hypospadias type and HOSE score (p=1.000), modified PPPS (p=0.500), or complication occurrence (p=1.000). Conclusions: GFC technique performed by a single operator provides satisfactory short-term functional and aesthetic outcomes in distal-glandular hypospadias with acceptable complication rates despite majority of patients operated at age above recommendations. Patient and family satisfaction with cosmetic results was very high. Surgical standardization through single operator demonstrates consistent excellent outcomes in a multicenter setting in Indonesia.
