KARAKTERISTIK DAN LUARAN PASIEN ANAK DENGAN SINDROM NEFROTIK RESISTEN STEROID DI RSUP DR. MOHAMMAD HOESIN PALEMBANG

Background. Steroid-resistant nephrotic syndrome (SRNS) represents a subtype of nephrotic syndrome associated with less favorable clinical outcomes compared to its steroid-sensitive counterpart. Despite its relatively low incidence, SRNS plays a substantial role in accelerating the progression toward chronic kidney disease and, ultimately, end-stage renal failure. In the Indonesian context, especially within the South Sumatra region, comprehensive data on the clinical profile and prognosis of pediatric patients affected by SRNS remain scarce and insufficiently explored. Methods. This descriptive retrospective study with a cross-sectional design used medical record data of pediatric SRNS patients from January 1, 2022 to December 31, 2024. Among a total of 324 nephrotic syndrome patients medical records, 71 patients were identified as SRNS, of whom 49 met the inclusion criteria. Results. The proportion of SRNS was 21.9% of all nephrotic syndrome cases. Most patients were aged 10–18 years (73.5%), male (57.1%), and had normal nutritional status (44.9%). Clinically, hypertension was found in 57.1% of patients and hematuria in 53.1%. The most common histopathological lesion was focal segmental glomerulosclerosis (73.5%). Cyclophosphamide was the most frequently administered therapy (61.2%). A total of 26 patients (53.1%) achieved remission, while 23 patients (46.9%) did not. The majority of patients were in chronic kidney disease stage I (77.6%), and no mortality was recorded (100% survival). Conclusion. Pediatric SRNS patients at Dr. Mohammad Hoesin Hospital Palembang were predominantly adolescents, with FSGS as the most frequent histopathological finding and cyclophosphamide as the most commonly used therapy. More than half of the patients achieved remission, and most were in early-stage chronic kidney disease, with no deaths during the study period.
