PENGUKURAN LINGKAR BETIS SEBAGAI PREDIKTOR SARKOPENIA PADA PASIEN LANJUT USIA DI POLIKLINIK GERIATRI RSUP DR. MOHAMMAD HOESIN PALEMBANG
Background: Sarcopenia is a condition characterized by a decrease in muscle mass accompanied by a reduction in muscle strength and/or physical performance, which frequently occurs in older adults and leads to an increased risk of falls, dependency, cognitive and cardiopulmonary function decline, lower quality of life, mortality, and high healthcare costs. Calf circumference is recommended as a simple screening method by the Asian Working Group for Sarcopenia (AWGS) 2019; however, the cut-off values recommended by AWGS 2019 are based on studies conducted in only seven Asian countries, namely China, Hong Kong, Japan, Singapore, South Korea, Taiwan, and Thailand. To date, no study has evaluated the ability of calf circumference as a predictor of sarcopenia in Palembang. Method: This study was a diagnostic test study with a cross-sectional design. The sample consisted of older adult patients who underwent examinations at the Geriatric Outpatient Clinic of Dr. Mohammad Hoesin General Hospital, Palembang, during the period 2023–2025 and met the inclusion criteria and were not included in the exclusion criteria. Sampling was performed using the total sampling method. The data used were secondary data obtained from medical records. Sarcopenia status was assessed based on the AWGS 2019 criteria, and calf circumference measurement was evaluated using diagnostic test analysis and ROC curve analysis. Results: This study analyzed 166 older adult patients, with the majority of subjects being female (63,9%), a median age of 70 years (60–88 years), and a median calf circumference of 30,25 cm (22–43 cm). A total of 88 patients were identified as having sarcopenia. The sensitivity, specificity, PPV, NPV, LR+, LR-, accuracy, and AUC were 98,864%, 51,282%, 69,600%, 97,561%, 2,029, 0,022, 76,506%, and 0,751, respectively. ROC curve analysis showed an optimal cut-off of ≤31 cm in men, with sensitivity, specificity, and AUC values of 91,89%, 73,91%, and 0,888, respectively, and an optimal cut-off of ≤29 cm in women, with sensitivity, specificity, and AUC values of 88,24%, 89,09%, and 0,943, respectively. Conclusion: Calf circumference has good ability as a predictor of sarcopenia in older adult patients, with high sensitivity and negative predictive value.
