HUBUNGAN SKOR LENT DENGAN PENAMBAHAN VOLUME EFUSI DAN KESINTASAN PADA PASIEN EFUSI PLEURA GANAS DI RS MOHAMMAD HOESIN PALEMBANG
Background: The presence of a malignant pleural effusion (MPE) indicates advanced stage in malignancy, which severely compromises in a poor prognosis. The LENT score has been developed as a prognostic tool for MPE, yet its association with pleural effusion progression and six-month survival has not been thoroughly evaluated, particularly in Indonesia. Methods: This prospective cohort study included 35 patients with MPE who underwent serial ultrasound examinations and LENT score assessments. Bivariate analysis, survival analysis using Kaplan-Meier curves, and Cox regression were used to assess variable associations. Results: The study cohort with a total of 35 study participants. High-risk LENT scores were observed in 31.4% of patients. There was a significant increase in pleural effusion volume between t1 and t2 (median Δvolume = 137.9 mL; p < 0.001). Kaplan-Meier analysis showed a median survival of 22 days for the high-risk group and 85 days for the moderate-risk group (p < 0.001). Reddish pleural fluid (p = 0.013) and lack of chemotherapy (p = 0.008) were also significantly associated with lower survival. Multivariate Cox regression revealed that a high-risk LENT score was the only independent predictor of six-month mortality (HR: 3.69; 95% CI: 1.37–9,99; p = 0.01). Conclusion: The LENT score is significantly associated with pleural effusion volume progression and six-month mortality in MPE patients. While pleural fluid color and chemotherapy status also affect survival, only the LENT score remained a significant predictor in multivariate analysis.
