Background: Sarcopenia is a progressive skeletal muscle disorder characterized by a decline in muscle strength, mass, and physical performance, leading to increased fall risks and reduced quality of life in the elderly. The pathogenesis is driven by "inflammaging," particularly elevated levels of Interleukin 6 (IL-6). Given the rising geriatric population in Palembang, early detection through community-based services such as the Elderly Posyandu is vital. This study aims to analyze the correlation between serum IL-6 levels and muscle mass, strength, physical performance, and calf circumference to establish a foundation for community-based sarcopenia management. Methods: This was an observational analytic study with a cross-sectional design. A total of 40 subjects were recruited via consecutive sampling based on predefined inclusion and exclusion criteria. Data were collected through interviews, questionnaires, and clinical measurements. Statistical analysis was performed using Pearson’s correlation for normally distributed data and Spearman’s rank correlation for non-normal distributions. Multivariate analysis was conducted using the backward regression method. Results: The study included 35 female (87.5%) and 5 male (12.5%) subjects aged ≥ 60 years. Bivariate analysis demonstrated that IL-6 levels were significantly correlated with muscle strength (p=0.004; r= -0,461), and physical performance (p=0.001; r=0,546). After adjusting for confounding variables, IL-6 levels remained significantly correlated with muscle strength and physical performance, but showed no significant correlation with muscle mass or calf circumference. Conclusion: Serum Interleukin-6 levels significantly correlate with muscle strength and physical performance among sarcopenic elderly individuals in the community health center setting of Palembang.
