PERBANDINGAN EFEKTIVITAS TAMPON TELINGA ANTIBIOTIK TUNGGAL DENGAN TAMPON TELINGA KOMBINASI ANTIBIOTIK STEROID PADA PASIEN OTITIS EKSTERNA DIFUSA AKUT

Background: Acute diffuse otitis externa is an inflammation of the skin and subcutaneous tissue of the external auditory canal, commonly caused by bacterial infection, particularly Pseudomonas aeruginosa and Staphylococcus aureus. Treatment involves ear canal cleaning, pain control, and topical antibiotic therapy using ear tampons. Single antibiotic tampons effectively treat infection but lack anti-inflammatory effects, whereas antibiotic-steroid combination tampons can reduce inflammation and pain more rapidly. This study aims to compare the effectiveness of these two tampon types in patients with acute diffuse otitis externa. Methods: This experimental study with a double-blind randomized controlled trial design was conducted on 40 patients with acute diffuse otitis externa classified as Brighton Grade II and III at Dr. Mohammad Hoesin Hospital Palembang during October-November 2025. Patients were divided into two groups receiving either single antibiotic (gentamicin) ear tampons or combination antibiotic-steroid (gentamicin- fluocinolone acetonide) ear tampons. Pain was evaluated using the Visual Analog Scale (VAS), and ear canal edema was assessed using the Brighton Grading at tampon insertion and follow-ups on days 1, 3, and 5. Results: The group receiving the combination antibiotic-steroid tampon showed a significantly faster and greater improvement in pain compared to the single antibiotic group (p = 0.034), although the difference in edema reduction did not reach statistical significance (p > 0.05). Conclusion: Ear tampons containing a combination of antibiotic and steroid are more effective in reducing pain in patients with acute diffuse otitis externa compared to single antibiotic tampons. The use of this combination is recommended to accelerate recovery and enhance patient comfort. Keywords: Acute diffuse otitis externa, ear tampon, antibiotic, steroid, pain, edema.
