Pulmonary tuberculosis (TB) remains a public health problem in the Benteng Primary Health Center catchment area, Central Bangka Regency. This study aimed to analyze the association of household crowding, home ventilation, and tobacco smoke exposure with pulmonary TB history and to identify the most dominant factor after controlling for confounders (age, education, occupation, HIV, and diabetes mellitus/DM). An analytic observational case–control design was conducted from October to December 2025, involving 118 respondents (59 cases and 59 controls; 1:1 ratio). Cases were defined as individuals with a confirmed history of pulmonary TB recorded in the TB register of Benteng Primary Health Center for 2023–2025, while controls were non-TB respondents. Bivariate analysis used the chi-square test to obtain crude odds ratios (ORs) and 95% confidence intervals (95%CIs), followed by logistic regression to obtain adjusted odds ratios (AORs) and assess confounding. Bivariate results showed significant associations between household crowding and pulmonary TB history ((OR=6,960; 95%CI: 2,590-18,699 p
