EVALUASI KEBERHASILAN PERAWATAN ORTODONTI CEKAT PADA KASUS PENCABUTAN PREMOLAR
Background: Malocclusion is commonly encountered conditions, with the global prevalence reaching 56% and up to 80% in Indonesia. One of the most frequent types of malocclusion is dental crowding. Moderate to severe dental crowding often requires premolar extraction to provide adequate space during fixed orthodontic treatment. Premolar extraction is a widely used strategy to improve dental alignment, enhance occlusal relationships, and optimize facial aesthetics. The success of fixed orthodontic treatment following premolar extraction is influenced by accurate diagnosis, appropriate treatment mechanics, patient compliance, and outcome evaluation based on Andrews’ six keys of occlusion and the PAR index. Objective: To evaluate the success of fixed orthodontic treatment in cases requiring premolar extraction. Methods: A narrative review using data sources from Google Scholar, PubMed, ScienceDirect, and ResearchGate published within the last 10 years (July 2015–July 2025). Results: A total of 18 studies were analyzed. All studies reported significant improvements in overjet, overbite, molar and canine relationships, correction of midline deviation, and enhancement of facial profile aesthetics. Several reports utilized the PAR index and showed a significant reduction in scores after treatment. Most cases achieved Andrews’ six keys of occlusion, particularly stable molar relationships and improved tooth rotation. Treatment duration varied depending on the severity of crowding and case complexity. Conclusion: Fixed orthodontic treatment with premolar extraction is proven effective in delivering functional, aesthetic, and stable treatment outcomes, especially in moderate to severe crowding cases.
