EFEK EKSTRAK CENTELLA ASIATICA TERHADAP PERANAN PERFORIN, GRANZIM B, DAN SEL DENDRITIK YANG DIHASILKAN PADA IMPLANTASI EMAS DI KELOPAK MATA KELINCI

Background: Gold implants are widely used in eyelid reconstruction but may trigger inflammatory responses through the activation of dendritic cells and cytotoxic T cells expressing perforin and granzyme B. This immune activation can lead to tissue damage and implant failure. Centella asiatica (CA) is known for its anti-inflammatory and immunomodulatory properties. However, its effects on dendritic cell, perforin, and granzyme B expression in gold implantation have never been evaluated. Method: This experimental in vivo post-test only control group study was conducted on 32 male New Zealand white rabbits. Two groups were formed: a control group (gold implant without CA) and a treatment group (gold implant + oral CA extract 75 mg/day for 7 days). Eyelid tissues were collected post-euthanasia and analyzed using immunohistochemistry to assess the density of dendritic cells, perforin, and granzyme B. Data were analyzed using the Shapiro–Wilk test and Mann–Whitney test. Result: CA extract significantly reduced the expression of all immunological markers. Mean perforin levels decreased from 20.5 to 8.91 cells/field (p = 0.023), granzyme B from 18.71 to 8.58 cells/field (p = 0.025), and dendritic cells from 1.35 to 0.16 cells/field (p = 0.039). These reductions indicate decreased activation of cytotoxic and antigen-presenting immune responses after CA administration. Conclusion: Centella asiatica extract effectively suppresses the expression of perforin, granzyme B, and dendritic cells in eyelid tissue with gold implants, demonstrating significant anti-inflammatory and immunomodulatory effects. CA shows potential as an adjuvant therapy to reduce immunological complications associated with gold implants.
