KORELASI EKSPRESI CLAUDIN 18.2 DENGAN SUBTIPE HISTOPATOLOGI ADENOKARSINOMA LAMBUNG

Background: Gastric adenocarcinoma remains a global health burden with high mortality, while effective targeted therapies are still limited. Claudin-18.2 is a transmembrane tight-junction protein selectively expressed in normal gastric mucosa and retained in gastric adenocarcinoma. Its role as a novel therapeutic target, including zolbetuximab, highlights the importance of investigating claudin- 18.2 expression, particularly in relation to histopathological subtypes of gastric adenocarcinoma. Methods: This was an analytic descriptive study with a cross-sectional design using archived hematoxylin–eosin slides and paraffin blocks of gastric adenocarcinoma cases from 1 January 2020 to 31 May 2025. Thirty-two samples were selected by non-random sampling. Clinicopathological data were obtained from medical records. Immunohistochemical staining was performed using a monoclonal antibody to claudin-18.2 on an automated Ventana Benchmark Ultra Plus platform. Claudin-18.2 expression was evaluated semi-quantitatively based on staining intensity and the proportion of positive tumor cells. Univariable, bivariable, and multivariable analyses were conducted; binary logistic regression with the backward LR method was used to identify predictors of claudin-18.2 expression. Results: Bivariable analysis showed a significant correlation between claudin-18.2 expression and the histopathological subtype of gastric adenocarcinoma (p = 0.017) with a moderate correlation strength (r = 0.423). Multivariable analysis confirmed that histopathological subtype (p = 0.023) and sex (p = 0.040) were independent predictors of positive claudin-18.2 expression. The highest predicted probability of positive claudin-18.2 expression was observed in the diffuse subtype among female patients (predicted probability = 0.945), showing an odds with odds approximately ≈17.2-fold higher likelihood of positive expression compared to the intestinal subtype in males. In contrast, age and tumor location showed no significant associations. Conclusion: Claudin 18.2 expression is significantly associated with histopathological subtypes of gastric adenocarcinoma, particularly in female patients with the diffuse subtype. These findings suggest that this subgroup represents a priority population for screening and potential claudin 18.2-based targeted therapy such as zolbetuximab.
