Pharmaceutical services are a critical component in ensuring the quality of hospital healthcare delivery. Waiting times that meet service standards reflect the efficiency of service processes as well as the performance of supporting systems, including human resources and information technology. This study aimed to comprehensively analyze barriers to service waiting times in the Outpatient Pharmacy Unit of RSUP Dr. Rivai Abdullah Banyuasin using the Donabedian framework and Lean Hospital principles. This research employed a mixed-methods approach with a sequential explanatory design. Quantitative data were obtained through service time measurements using a Time and Motion Study (TMS) on a total of 341 prescriptions, consisting of 249 non-compounded medications and 92 compounded medications. Qualitative data were collected through in-depth interviews with pharmacists, pharmacy technicians, and hospital management. The results showed that the average waiting time for non-compounded medications was 24,61 minutes, with 72,29% meeting the standard of ≤ 30 minutes, while the average waiting time for compounded medications was 32,57 minutes, with 94.57% meeting the standard of ≤ 60 minutes. The longest stage occurred during the medication preparation process, which is categorized as a value-added (VA) activity and dominated the overall service duration. Barriers to service waiting times were primarily caused by limited human resources during peak prescription volumes arriving simultaneously from outpatient clinics, network disruptions, rationalization of prescribing , and errors in depot data entry. The medication preparation stage generated the longest service time, indicating the need for optimization of human resources, strengthening of the hospital management information system, improvement of service workflows to enhance service quality, and reduce waiting time duration.
