PERBANDINGAN SURVIVAL RATE PASIEN ANAPLASTIC LARGE CELL LYMPHOMA SUBTIPE ALK-POSITIF DAN ALK-NEGATIF DI RSUP DR. MOHAMMAD HOESIN PALEMBANG

Background. Anaplastic Large Cell Lymphoma (ALCL) is a mature T-cell lymphoma characterized by strong CD30 expression and is classified into ALK-positive and ALK-negative subtypes. Although both subtypes share similar histopathological features, differences in ALK expression are associated with distinct biological behavior, treatment response, and prognosis. International studies have reported better survival rates in patients with ALK-positive ALCL; however, local data in Indonesia remain limited. This study aimed to compare the survival rates of patients with ALK-positive and ALK-negative ALCL at Dr. Mohammad Hoesin General Hospital Palembang. Methods. This study was a retrospective descriptive comparative study using a total sampling method. The sample included all patients diagnosed with ALK-positive and ALK-negative ALCL from January 1, 2016 to December 31, 2024. Data were obtained from medical records, immunohistochemistry reports for ALK expression, and family interviews when survival status was unavailable. Survival analysis was performed using the Kaplan-Meier method and differences between groups were analyzed using the log-rank test. Results. A total of 13 patients were included, consisting of 7 (53.8%) ALK-positive and 6 (46.2%) ALK-negative cases. During the observation period, 2 deaths (15.4%) occurred, all in the ALK-negative group. The Kaplan-Meier curve descriptively showed better survival in the ALK-positive group; however, the log-rank test demonstrated no statistically significant difference between the two groups (p = 0.067). Conclusion. There was no statistically significant difference in survival rates between patients with ALK-positive and ALK-negative ALCL, although descriptively the ALK-positive group showed better outcomes.
