Diarrhea remains a public health problem globally and in Indonesia. Based on 2025 data, diarrhea cases at Puskesmas Nagaswidak were the highest in Seberang Ulu II District. Clean and Healthy Lifestyle Behavior (CHLB), particularly handwashing with soap behavior (HWWS), plays an important role in preventing diarrhea. This study aimed to analyze the association between mothers’ handwashing with soap behavior and the incidence of diarrhea in the working area of Puskesmas Nagaswidak, Palembang. This study used a quantitative analytic approach with a cross sectional design. The population consisted of all mothers with children aged 1–59 months in the working area of Puskesmas Nagaswidak with a total of 97 respondents selected through proportional random sampling. Data were collected through questionnaire-based interviews and observation and were analyzed using univariate, bivariate using the Chi-Square test, and multivariate analysis. The results showed that mothers’ handwashing with soap behavior after defecation (p-value=0.010; PR=1.748; CI=1.131–2.703), before cooking (p-value=0.036; PR=1.949; CI=0.983–3.865), and before feeding children (p-value=0.020; PR=1.970; CI=1.043–3.721) were significantly associated with the incidence of diarrhea in under-five children, whereas maternal age (p-value=0.298), availability of handwashing facilities (p-value=0.754), and cleanliness of eating utensils (p-value=1.000) were not associated with the incidence of diarrhea in under-five children. The dominant factor influencing the incidence of diarrhea was mothers’ handwashing with soap behavior after defecation (p-value=0.042; OR=2.509; CI=0.889–7.081). It is recommended that the Puskesmas increase health education and outreach for mothers with under-five children to pay more attention to handwashing with soap practices. Keywords : Diarrhea, Hand Washing With Soap (HWWS), Under-Five Children, CHLB, mother’s behavior
