PENGARUH PEMBERIAN SUPLEMEN VITAMIN D TERHADAP KADAR BRAIN DERIVED NEUROTROPHIC FACTOR DAN PERBAIKAN FUNGSI KOGNITIF PADA PASIEN USIA LANJUT DENGAN GANGGUAN KOGNITIF RINGAN DI INSTALASI RAWAT JALAN GERIATRI RSUP DR MOHAMMAD HOESIN PALEMBANG
Background: Aging is often accompanied by cognitive decline, where Mild Cognitive Impairment (MCI) serves as a transitional phase at risk of developing into dementia. The pathophysiology of this condition is closely related to neurobiological mechanisms, including low levels of Brain-Derived Neurotrophic Factor (BDNF) and the high prevalence of Vitamin D insufficiency in the elderly population. This study aims to evaluate the effect of Vitamin D supplementation on increasing BDNF levels and improving cognitive function in geriatric patients with MCI. Methods: This study was a double-blind randomized clinical trial involving 30 geriatric patients with MCI at Dr. Mohammad Hoesin General Hospital, Palembang, from August 2025 to January 2026. Subjects were randomized into two groups to receive either 5000 IU Vitamin D supplementation (n=15) or placebo (n=15) for 12 weeks. Assessments were performed on serum 25(OH)D levels, BDNF levels, and cognitive function using the Indonesian version of the Montreal Cognitive Assessment (MoCA-Ina) score. Results: Vitamin D supplementation significantly increased the mean serum 25(OH)D levels, reaching a sufficiency level of 44.25 ng/mL (p < 0.001). In the treatment group, Vitamin D supplementation successfully inhibited the decline of serum BDNF levels, differing significantly from the placebo group which experienced a drastic decline (p < 0.001). Global cognitive function (MoCA-Ina) improved in the Vitamin D group with a median score increase of 2.0 points (p = 0.002), in contrast to the decline observed in the placebo group. Multivariate analysis confirmed that Vitamin D supplementation, marital status, and comorbidity score (CIRS-G) were independent factors influencing changes in cognitive scores (Adjusted R Square = 0.652). Conclusion: Vitamin D supplementation of 5000 IU for 12 weeks was proven effective in inhibiting the rate of decline in serum BDNF levels and improving cognitive function in geriatric patients with MCI. This intervention demonstrates a good safety profile and can be considered as a therapeutic strategy in the management of elderly patients.
