HUBUNGAN STATUS IMUNISASI DIFTERI-PERTUSIS-TETANUS (DPT) DENGAN KEJADIAN PERTUSIS PADA BAYI DAN BALITA DI KOTA PALEMBANG TAHUN 2023–2025

Background: Pertussis remains a health problem among infants and children under-five despite the ongoing Diphtheria–Tetanus-Pertussis (DTP) immunization program. Objective: To assess the association between DPT immunization status and PCR-confirmed pertussis among infants and children under-five in Palembang City, 2023–2025.Methods: A test-negative design study was conducted using secondary pertussis surveillance data (PERT-02 line list). The source population included children aged 2–60 months with suspected pertussis symptoms who were reported to the surveillance system and tested by PCR during 2023–2025. A total of 134 children were included in the analysis. Cases were symptomatic children with positive pertussis PCR results (n=58), while controls were symptomatic children with negative PCR results (n=76). The main exposure was DTP immunization status (unvaccinated, partially vaccinated, and up-to-date [UTD]). Data were analyzed using logistic regression. Results: The proportion of PCR-confirmed pertussis was highest among unvaccinated children (53.52%) and lowest among those with up-to-date DTP immunization (24.14%). After adjustment for age, up-to-date DTP immunization was associated with lower risk of pertussis pertusis (aOR=0,0,307; 95% CI 0,113 – 0,833), corresponding to an estimated vaccine effectiveness (VE) of 69,3% (95% CI VE 16,7% – 88,7%). Partial vaccination showed a protective direction but was not statistically significant (aOR=0,589; 95% CI 0,251 – 1,386). Spatially, PCR-positive cases were reported both in primary health center catchment areas with low DTP coverage and in those that had reached the target, suggesting possible pockets of susceptible children. Conclusion: Up-to-date DTP immunization provided significant protection against pertussis. Strengthening on-time vaccination and catch-up immunization, particularly in early infancy, is recommended.
