Introduction:Breast carcinoma is the most common breast malignancy among women in Indonesia,accounting for 30.8% of female cancer cases in 2020, with high prevalence at Dr. Mohammad Hoesin General Hospital (20.9% in 2019–2020). Serum calcium plays a crucial role in cellular regulation of cancer through homeostasis of PTH, vitamin D, and mechanisms such as PTHrP and local osteolysis, where hypercalcemia is often associated with large tumors due to bone destruction or ectopic production. Methods:This descriptive case series study aimed to analyze the relationship between tumor size (cut-off 5 cm) and serum calcium level (normal range 8.5–11 mg/dL) in 35 stage III patients before neoadjuvant chemotherapy, using consecutive sampling during January–February 2026. Data were obtained from medical records; serum calcium was analyzed spectrophotometrically (arsenazo III method), tumor size determined by ultrasound/mammography, and tested using Mann-Whitney U and Shapiro-Wilk via STATA 15. Results: The mean age was 50.5 years; most patients had Luminal B type (77.1%), tumor size >5 cm (77.1%), and normal calcium levels (68.57%), with hypocalcemia in 22.86% and hypercalcemia in 8.57%. There were significant differences in calcium levels (p=0.006) and tumor size (p=0.000) between Luminal A and Luminal B IHC types. Mean calcium levels were 8.54 mg/dL (5 cm), with a strong correlation (r=0.731, p=0.000). Conclusion:A significant relationship was found between tumor size and serum calcium, supporting its potential as a biomarker for predicting therapeutic response and prognosis. Recommendations include routine calcium screening, further research involving vitamin D/PTHrP, and integration into Dr. Mohammad Hoesin Hospital’s clinical protocols.
