ANALISIS KESINTASAN KEJADIAN SURGICALLY INDUCED ASTIGMATISM PADA PASIEN PASCA FAKOEMULSIFIKASI DENGAN PENDEKATAN DARI ARAH TEMPORAL PADA PASIEN ASTIGMATISME REGULER
Background: Corneal incisions alter the curvature of the corneal surface, leading to surgically induced astigmatism (SIA). Temporal incisions are generally superior to superior incisions due to improved surgical access and lower SIA values. This study aims to analyze the survival of SIA values resulting from temporal corneal incisions in cataract patients undergoing phacoemulsification. Methods: A non-comparative clinical trial using survival analysis was undertaken at the Eye Clinic of Palembang Eye Specialty Hospital from September 2024 to April 2025. Thirty-six cataract patients with regular astigmatism who underwent phacoemulsification were included based on inclusion criteria. SIA values were evaluated at day 1, month 1, and month 3 postoperatively. Survival analysis was performed using the Kaplan-Meier method with SPSS version 26.0. Results: The study revealed a significant reduction in SIA values at day 1, month 1, and month 3 in both the Against-the-Rule (ATR) group (p = 0.000) and the Withthe- Rule (WTR) group (p = 0.001). Furthermore, significant differences in SIA values were observed between ATR and WTR at day 1 (p = 0.004), month 1 (p = 0.010), and month 3 (p =0.000). There was a significant difference in the estimated improvement of SIA between WTR and ATR patients (p = 0.005). ATR type patients showed a 52.6% higher probability of SIA improvement compared to WTR patients, though this was not statistically significant (HR = 1.526; p = 0.276). Conclusion: Patients with ATR astigmatism exhibit lower SIA values and a shorter estimated time for SIA improvement compared to patients with WTR astigmatism.
