HUBUNGAN KEBUTUHAN CONTINUOUS RENAL REPLACEMENT THERAPY (CRRT) DENGAN MORTALITAS PADA PASIEN ACUTE KIDNEY INJURY (AKI) DI INTENSIVE CARE UNIT (ICU)RS MOHAMMAD HOESIN PALEMBANG

Background: Acute Kidney Injury (AKI), the most serious complications in critically ill patients admitted to the Intensive Care Unit (ICU). Under these conditions, the required renal replacement therapy modality that can be performed in the ICU is Continuous Renal Replacement Therapy (CRRT). However, the mortality rate remains high in patient with CRRT. This study aims to analyze the relationship between the requirement for CRRT and mortality in AKI patients in the ICU. Methods: This was a retrospective cohort study involving 30 AKI patient’s treated in the ICU of Mohammad Hoesin Hospital Palembang from January 1st, 2023 until December 30st, 2025. Medical record data were collected to assess the indications for CRRT, mortality, and confounding variables. The data were analyzed using Fisher’s exact test. Results: The mortality rate among AKI patients in the ICU requiring CRRT at Mohammad Hoesin Hospital Palembang between January 1, 2023, and December 30, 2025, was 86.7%. There was no significant relationship between the need for CRRT and mortality (p > 0.05). Discussion: This study found that neither the type of CRRT indication nor the number of underlying causes for CRRT requirements was significantly associated with mortality. This indicates that once an AKI patient in the ICU meets the requirement for CRRT initiation, whether due to a single severe indication such as acidosis or a combination of indications, their risk of death has reached an equivalent level of vulnerability. Conclusion: The requirement for CRRT is not significantly associated with mortality because the clinical conditions that necessitate a patient to receive CRRT inherently indicate that the patient is already at a high risk of death.
