Anemia among adolescent girls remains a public health problem that can affect learning concentration and future reproductive health. The Iron Folic Acid (IFA) supplementation program has been implemented as an effort to prevent anemia; however, adherence to IFA consumption remains low. Positive and negative knowledge regarding IFA is suspected to be associated with adherence to its consumption. This study aimed to analyze the relationship between positive and negative knowledge about IFA and adherence to its consumption among adolescent girls at SMP Negeri 1 Tanjung Agung, Muara Enim Regency. This research employed an analytical observational design with a cross-sectional approach. The sample consisted of 107 adolescent girls selected using probability sampling with a proportionate stratified random sampling technique. Data were collected using a structured questionnaire and analyzed using univariate and bivariate analysis with the Chi-Square test at a significance level of 0.05. The results showed that the majority of respondents were aged 14–15 years (66.4%). Positive knowledge was categorized as good in 50.5% of respondents and poor in 49.5%, while negative knowledge was categorized as good in 29.0% and poor in 71.0%. The proportion of respondents who adhered to IFA consumption was 23.4%. Statistical analysis indicated a significant relationship between positive knowledge (p-value = 0.001) and negative knowledge (p-value = 0.001) with adherence to IFA consumption among adolescent girls at SMP Negeri 1 Tanjung Agung, Muara Enim Regency. The level of positive and negative knowledge about TTD is significantly associated with compliance in consumption among adolescent girls. To improve compliance, schools can implement stricter supervision by organizing regular weekly TTD consumption activities.
