EFIKASI SUPLEMENTASI EKSTRAK IKAN GABUS TERHADAP KENAIKAN BERAT BADAN DAN PERBAIKAN PROTEIN PENANDA INFLAMASI FASE AKUT PADA ANAK GIZI BURUK: SEBUAH STUDI PENDAHULUAN
Severe acute malnutrition (SAM) remains a global health challenge; Indonesia's wasting rate reached 8.5% in 2023, surpassing the national target of 7%. Children with SAM experience depleted visceral protein stores, including prealbumin, and heightened systemic inflammation reflected by elevated high-sensitivity C-reactive protein (hs-CRP), each condition mutually exacerbating the other. Snakehead fish extract (Channa striata) is rich in high-quality protein, essential amino acids, zinc, and highly bioavailable albumin, offering potential to restore visceral protein status and attenuate inflammation. This double-blind randomized controlled pilot trial enrolled 30 children aged 1–5 years with SAM at the NPM Clinic of RSMH and community health centers in Palembang (December 2024–January 2026). The intervention group (n=15) received one sachet of snakehead fish extract (Inbumin Forte®) daily, while the control group (n=15) received placebo for 14 consecutive days; both groups received standard SAM nutritional management. Body weight, prealbumin, and hs-CRP were measured at baseline and post-intervention and analyzed using intention-to-treat (ITT) and per-protocol (PP) approaches. In the PP analysis, the intervention group showed a mean prealbumin increase of 9.19%, whereas the placebo group experienced a significant decrease of 11.68% (p=0.039); the between-group difference in mean prealbumin change was statistically significant (p=0.035). A greater proportion of subjects in the intervention group achieved prealbumin improvement (63.6% vs 15.4%; RR=4.136; 95% CI: 1.071–15.969; p=0.033). Both groups gained weight significantly with no significant between-group difference (p>0.05). A trend toward hs-CRP reduction was more frequent in the intervention group (54.5% vs 15.4%), although statistical significance was not reached (p=0.082). Fourteen-day snakehead fish extract supplementation in children with SAM significantly preserved and increased prealbumin levels compared to placebo, with a favorable trend toward hs-CRP reduction. Larger studies with longer intervention periods are needed to confirm these findings. Keywords: severe acute malnutrition, snakehead fish extract, prealbumin, hs-CRP
