HUBUNGAN PENGGUNAAN OBAT ANTI BANGKITAN DENGAN KETEBALAN TUNIKA INTIMA MEDIA ARTERI CAROTIS PADA PASIEN EPILEPSI DI POLIKLINIK NEUROLOGI RS MOHAMMAD HOESIN PALEMBANG
Background: Epilepsy is a chronic condition requiring long-term antiepileptic drug (AED) therapy. Prolonged use of AEDs, particularly first-generation drugs, has been reported to increase the risk of cardiovascular and cerebrovascular events through the process of atherosclerosis. Carotid artery intima-media thickness (IMT) is a sensitive indicator for assessing atherosclerosis. This study aimed to analyze the association between AED use and carotid artery IMT in epilepsy patients at the Neurology Outpatient Clinic of Mohammad Hoesin Hospital, Palembang. Methods: This was an observational analytic study with a case-control design. A total of 34 subjects were included, consisting of 17 epilepsy patients who had received AED therapy for more than 6 months as the case group, and 17 healthy individuals matched by age and sex as the control group. Carotid artery IMT was measured using Carotid Doppler ultrasonography. Results: All subjects were aged between 20–40 years, with an equal distribution of sex between groups. In the case group, the most commonly used AED was carbamazepine (44.4%), with the majority having used AEDs for more than 24 months (70.6%). The mean carotid IMT in the case group was 0.442 ± 0.027 mm (median 0.430 mm), while in the control group it was 0.457 ± 0.044 mm (median 0.455 mm). Mann–Whitney test showed no statistically significant difference between the two groups (p = 0.545). Conclusion: There was no significant association between AED use and carotid artery IMT in epilepsy patients at the Neurology Outpatient Clinic of Mohammad Hoesin Hospital, Palembang. The absence of lipid profile and inflammatory marker assessments, as well as differences in body mass index distribution between groups, may have contributed to these findings. Further longitudinal studies with more comprehensive control of confounding factors are needed to confirm this association.
